MyChart™ is back online for Cornwall Hospital as of May 15, 2024. EXISTING USERS DO NOT NEED TO REGISTER AGAIN. IF YOU ALREADY have an account, SIMPLY LOG IN TO FIND YOUR RECORDS RESTORED. IF YOU ARE NEW TO MYCHART™, PLEASE PROCEED WITH REGISTERING.

 

 

Registration Form

This form will help you register for your MyChart™ account. To activate your account, you’ll need a four-digit PIN.

This PIN is a one-time-use registration number and is required to create a username and password for your account. You will receive an email from mychart@sunnybrook.ca with a link to the MyChart™ portal to complete the registration process. Your PIN must be used within 10 days of receipt, otherwise you’ll have to contact the MyChart™ Support Line at 1-833-221-2202.
