The Geriatric Mental Health Services
The Geriatric Mental Health Service of the Cornwall Community Hospital is a bilingual team that provides service to persons living in the community of Stormont, Dundas, Glengarry, and Akwesasne. There are 2 components to this service:
- A Community Team which sees individuals living in their homes or in a retirement home setting, and
- the Outreach Team, which sees individuals living in Long Term Care Facilities (nursing homes).
- Referral
Individuals 65 years of age and older with a current or suspected mental health problem will receive an initial assessment and follow-up, including counseling, as well as case management. Support and education is also provided to their families, while working collaboratively with other care providers. The main reasons people access this service is for help with memory problems and/or problems with mood. The service is voluntary and confidential, and can be provided either in office, or wherever the client lives.
Geriatric Psychiatrists from the Royal Ottawa Hospital are on-site weekly, and provide assessment and consultation to the client’s primary care provider (family doctor or nurse practitioner). The Community Service also provides education to community groups on mental health issues in the elderly.
