Cornwall Community Hospital Expands its Digital Portfolio by Adding SeamlessMD for Pre and Post-Surgical Monitoring
2022-03-14
Cornwall
Community Hospital (CCH) has launched SeamlessMD, a Digital Patient Engagement
platform used by hospitals and health systems across North America, to improve
the surgical patient experience.
The hospital
will implement SeamlessMD across several surgical service lines including Total
Hip and Knee Replacement, Shoulder, Breast, Colorectal, and for all Emergent
General Surgery.
With SeamlessMD,
patients undergoing surgery at CCH are guided from pre-surgical preparation
through post-surgical recovery via personalized education, progress-tracking,
and post-op symptom monitoring using their own device (e.g. smartphone, tablet,
computer).
After surgery
while recovering at home, patients can access education and self-report data
such as pain scores, symptoms, range-of-motion, and share photos of their
wounds through the platform remotely.
Providers at CCH
can monitor patients on dashboards allowing for personalized care
recommendations and faster intervention from the surgical care team if
necessary.
Increased
engagement with surgical patients through SeamlessMD will also help to reduce
hospital length of stays, readmissions and Emergency Department visits.
“With
SeamlessMD, patients are empowered to take charge of their own care, as the
platform provides the tools to effectively self-manage at home,” explains Dr.
Chris Raynor, Orthopedic Surgeon at CCH. “This can help patients feel less
anxious, more engaged, prepared and connected to their care team.”
CCH will also be
the first hospital in Canada to integrate SeamlessMD with its Cerner Electronic
Health Record (EHR) using SMART on FHIR technology – the latest EHR
interoperability framework. This integration will allow CCH to streamline
patient enrollment and monitoring on SeamlessMD from right within the Cerner
patient chart.
“SeamlessMD is
an important technology that aligns well with our mission to provide an
exceptional care experience for every patient. Its implementation, as well as
CCH being the first hospital in Canada to integrate it with its electronic
health record, also highlights the hospital’s commitment to being an innovative
leader in health care delivery,” adds Dr. Raynor.
“We are excited
to collaborate with CCH as the hospital continues to demonstrate exemplary
leadership in healthcare innovation,” says Dr. Joshua Liu, CEO at SeamlessMD.
“As we navigate through the COVID-19 pandemic, we remain dedicated to
protecting the health and safety of our community through the delivery of effective
virtual care, and are grateful for the ongoing support provided by Ontario
Health.”
