The Geriatric Day Hospital
The Geriatric Day Hospital is a specialized geriatric service funded by the Regional Geriatric Program of Eastern Ontario providing comprehensive multidisciplinary geriatric assessments for older adults, 65 years old and over, who are experiencing a change in function, mobility, memory, mood, or have complex medical issues.
Referrals are accepted from family physicians, health care professionals and community agencies. Referrals should be faxed directly to us. (ER physician may request a GEM assessment). Urgent assessments are available.
Services available at the Geriatric Day Hospital:
- Individual assessment with a Nurse Practitioner, a Physiotherapist, an Occupational Therapist & a Mental Health Counselor.
- Cognition Clinic with a physician specializing in Care of the Elderly
- Geriatric Day Hospital Group -2 mornings a week for 7-8 weeks. Clients must be able to manage their own continence, medically stable enough to participate in an exercise program and require the services of at least two of our disciplines.
- Caregiver Information Sessions
Our target population includes people who are at risk for falls, decreased function, memory/mood issues, isolation, safety concerns and/or general deconditioning.
Through individual assessment and treatment and/or group participation, we strive to support the person in the community by promoting healthy living, achieve/maintain a level of independence, provide physical and cognitive exercise, encourage socialization, provide support and education and address medical, functional and social issues.
A summary of group interventions and recommendations for future management are forwarded to the primary care provider and the referral source at the end of the program.
