MyChart™ is Back Online for CCH

  • All patient records accessible through MyChart™ previously have been restored to their former availability.
  • Health records generated during the MyChart™ offline period are available.

What is MyChart™?

MyChart™ is a free, secure online tool providing patients, their designated family members, and caregivers at CCH access to medical records and personal health information. Using your computer or smart device, MyChart™ users can easily view test results, clinical documentation, and other health information. MyChart™ was developed by Sunnybrook Health Sciences Centre in Toronto and is used by over 40 hospitals across Ontario. Currently thousands of patients at CCH are registered for and using MyChart™ to remotely access their personal health information. 


 Patients using MyChart™ can:

  • Access medical and personal health information in one secure online portal.
  • Share access to medical and personal health information with multiple healthcare professionals, family members or caregivers, keeping the right people informed.
  • Take control of health information no matter where they are, accessing tools for self-monitoring and tracking, appointments, vaccinations records, and reminders for annual screenings like colonoscopy, mammograms, and physicals.
  • Access Ontario lab test results directly within MyChart.

Signing up is easy!

Before getting started, you will need:

  1. An email address and access to the internet.
  2. An Ontario or Quebec health card.  *Patients must be sixteen years of age or older to sign up for MyChart™.

There are two ways to sign up for MyChart™

  1. If you are new to MyChart™, fill out the MyChart registration form on our website; or
  2. When registering for your next visit at CCH.

MyChart™ Technical Support:

  • Are you waiting for your registration pin? Registration pins are not generated instantaneously. It could take up to 5 to 10 business days for you to receive your pin from the mychart@sunnybrook.ca email address after you register. If you have still not received your pin after 10 days, be sure to check your junk/spam folder in your email inbox.
  • If you are still having technical issues or problems registering, please call the MyChart™ Support Line managed by Sunnybrook Health Sciences Centre at 1-833-221-2202. Please do not call the hospital.
  • We are currently experiencing higher than normal registration volumes. It may take longer than 5 to 10 days for registration pins to be emailed once you have completed the registration form. Thank you for your patience.

Frequently Asked Questions

Where do I go online to access MyChart™?

MyChart™ is accessible from anywhere at any time through the Internet. Once the registration process is complete, users can go to the MyChart website, and log in to review their records.

How can I grant others access to my MyChart™ profile?

When you first register for MyChart™, only you will be able to see your health records.

After you register, you can choose to electronically grant access to family, caregivers, and primary care providers, through the “Share My Records” feature within MyChart™. Similar to an online banking e-transfer, you will be required to provide a valid email and specify a secret keyword that your delegate must successfully enter, upon receiving an email invite, to gain access to the information you have authorized.

What information will I have access to through MyChart™?

With MyChart™, you can view hospital-acquired medical documentation online on a computer, laptop, tablet, or cell phone at any time of day. This information includes:

  • Diagnostic imaging reports (i.e. CT, MRI, X-Ray, Ultrasound)
  • Discharge summaries
  • Consultation notes
  • Emergency notes
  • Operative reports
  • History and physicals
  • Ontario lab results
You can also self-manage other health information not acquired through the hospital to customize the portal as a personal health record. You can input your:
  • Medications
  • Allergies
  • Immunizations
  • Procedures
  • Chronic conditions
  • Height and weight
  • Mood
  • Upcoming scheduled appointments

 

 

How does MyChart™ receive its information?

Your information is sent to MyChart™ after doctors and health care professionals have enough time to complete their documentation.

How does MyChart™ make sure personal information stays safe and confidential?

MyChart™ is secure like online banking; it uses the same encryption technologies used by the major banks.

I signed up for an account, but I didn't get an activation email. What do I do?

It could take 5-10 business days for you to receive the email from mychart@sunnybrook.ca

 

If you did not receive the email with a four-digit PIN within 10 days, please check your spam/junk folder. If you have checked and haven't received the email, please call the Sunnybrook MyChart™ support line at 1-833-221-2202.

The Sunnybrook MyChart™ Support Line hours of operation are Monday to Friday from 08:30 – 04:30. Response time when leaving a voicemail message is within 24 -48 hours within their hours of operation.

 

I logged into MyChart™ but my test results aren't there. Why?

Results are subject to a delay to provide doctors and health care professionals with enough time to complete all required documentation. Records will be made available as soon as clinicians have validated your information.

If you are looking for an older test result, please note that only test results and medical notes generated after May 8, 2020, are included in MyChart™.

If some of my health information on MyChart™ is not correct, what should I do?

Making changes and/or corrections to your medical record will ensure the quality of your record is maintained. This can be done by contacting Health Information Services at 613-938-4240 ext. 2247.

I have a question about my results - what do I do?

For questions about test results or reports, please contact your healthcare provider.

Can a power of attorney obtain a PIN on my behalf?

Instructions to obtain a PIN on behalf of a patient will be provided by contacting our Release of Information Office at 613-938-4240 ext. 2247.

What happens if I lose or forget my password?

If you lose or forget your password, you can reset it by clicking the “forgot password” link on the MyChart login page, or by calling the MyChart™ Support Team at 1-833-221-2202.

If I update my personal information (i.e. email, home address, telephone number) on MyChart™, does my information automatically update at CCH?

No, edits you make to your personal details on MyChart™ will not update automatically at the hospital. To update your personal information at CCH, please contact our Patient Registration Department. They can be reached by calling 613-938-4240 ext. 4060.

If I am signed up for MyChart™ in Ottawa, will I be able to access all of my health information in the same place?

CCH's MyChart™ is different than The Ottawa Hospital's online tool, which is specific to their electronic record.

Developed by Sunnybrook Health Sciences and recognized by the Ontario Ministry of Health, CCH's MyChart™ allows health information to be shared across all hospitals and healthcare partners in its system.
