Complete this form to receive your MyChart™ registration PIN to activate your account. Your PIN and instructions for account activation will be emailed to you from mychart@sunnybrook.ca within 5 to 10 business days after completing this form.

Your PIN must be used within 10 days of receipt. If not, you will need to contact the MyChart™ Support Line at 1-833-221-2202.

If you do not receive the email with your PIN within 10 days after completing this form, please check your spam/junk folder. If you still cannot find it, call the Sunnybrook MyChart™ Support Line at 1-833-221-2202.

The support line operates Monday to Friday from 08:30 to 16:30. Response time for voicemail messages is within 24-48 hours during operating hours.
