Transitional Day Program

The Transitional Day Program provides intensive and structured services to the severely mentally ill, 16-years of age and over, with the following objectives:

• To provide stabilization and support via individual and group intervention.
• To empower the individual to live a more satisfying and independent life in the community
• To determine and arrange for referrals to and linkages with other services and programs, including discharge planning.
• To provide support and education to families and consultation to other agencies involved with the patient
