Community Addiction and Mental Health Services
What is "On Track"?
“On Track” is part of the Cornwall Hospital’s Community Addiction and Mental Health Services. We work closely with The Champlain District Regional First Episode Psychosis program that is based out of Ottawa. It provides early intervention and treatment for psychosis. The goal of treatment is remission of symptoms and community re-integration. We help clients improve their well-being through the use of effective medications and other interventions, such as psycho-therapy, education, individual and family support. Community agencies are partners in care and help assist with things such as vocation and support groups. These approaches promote recovery and prevent relapse.
Who should be referred?
- People between the ages of 16 and 35 who may be having early symptoms or signs of psychosis.
- People who are experiencing their first psychotic episode.
- People who have a psychotic illness and have not been treated for more than six months.
